A joint injection should follow a working diagnosis, evidence review and informed consent, not be chosen only by a product name or advertised price.

An injection placed inside a joint

Intra-articular injections may be discussed for selected joint conditions, commonly involving the knee and sometimes the shoulder or hip. The purpose varies: reduce short-term inflammation, support participation in rehabilitation, or consider another biological or viscosupplement option after reviewing evidence and alternatives.

Pain around a joint is not always coming from inside the joint. Tendon, bursa, nerve, muscle, fracture, infection or referred spine pain may require a different treatment.

Corticosteroid, PRP and “gel” are different treatments

Corticosteroid

May provide short-term relief in selected osteoarthritis or inflammatory presentations. It does not rebuild cartilage and repeated use requires careful risk review.

Platelet-rich plasma (PRP)

Prepared from the patient’s blood. Evidence, preparation methods, cost and response vary; it should not be presented as guaranteed cartilage regrowth.

Hyaluronic acid / gel

Also called viscosupplementation. Major guidelines do not recommend routine use for knee osteoarthritis, so uncertainty, cost and alternatives need explicit discussion.

Aspiration and analysis

Removing joint fluid may help diagnosis or symptoms in selected swollen joints. Suspected infection or crystal disease changes the pathway.

Guidance can support accurate placement in selected joints

Musculoskeletal ultrasound may help visualise the target, avoid nearby structures, confirm fluid and document needle placement. It does not make an unsuitable treatment appropriate, and it does not replace sterile technique, consent or a rehabilitation plan.

Hip and some deeper or technically difficult targets are commonly considered for image guidance. The need depends on the joint, anatomy, diagnosis and clinician judgement.

Important questions before a joint injection

  • What diagnosis is being treated, and what alternatives are reasonable?
  • What benefit is realistic, for how long, and what does the evidence say?
  • Is there diabetes, infection risk, allergy, pregnancy, anticoagulant use or another factor affecting safety?
  • What medicine or preparation is being used, and is imaging guidance planned?
  • What rehabilitation and activity plan follows the procedure?
Do not inject a suspected infected joint. Fever, feeling unwell and a new hot, red, severely painful swollen joint needs urgent medical assessment.
