PM&R means Physical Medicine and Rehabilitation
A PM&R physician, also called a physiatrist or rehabilitation-medicine doctor, is a medical doctor trained to diagnose and manage conditions affecting function. The specialty covers the brain, spinal cord, nerves, muscles, bones, joints, tendons and the effects of illness or injury on daily life.
In Pakistan, patients may search for an orthopaedic doctor, neurologist, neurosurgeon, pain specialist, spine doctor or physiotherapist because “PM&R physician” is less familiar. These professionals have different training. PM&R often provides a useful non-surgical starting point when pain, nerve symptoms and loss of function overlap.
Problems involving pain, movement, nerves and independence
Back, neck and joint pain
Sciatica, disc-related symptoms, arthritis, shoulder or knee pain, tendinopathy and sports injury when non-surgical evaluation is appropriate.
Nerve and muscle symptoms
Numbness, tingling or weakness that may need neurological examination and selected EMG/NCS testing.
Neurological rehabilitation
Stroke, spinal cord injury, cerebral palsy, brain injury and other conditions affecting mobility or self-care.
Complex recovery
Function after surgery, prolonged illness, injury, amputation or chronic pain involving several body systems.
Diagnosis and function are considered together
A consultation reviews symptoms, medical history, medicines, previous tests and the activities that have become difficult. Examination may include the spine and joints, strength, sensation, reflexes, gait, balance and functional tasks.
The plan may use education, medication review, rehabilitation, activity progression, orthoses or equipment, EMG/NCS, musculoskeletal ultrasound and selected image-guided procedures. Not every patient needs every component.
PM&R works with, not against, other specialties
- Orthopaedics or neurosurgery: fracture, instability, severe structural disease or a likely surgical indication.
- Neurology: diseases of the brain, spinal cord, nerves or muscles needing neurological diagnosis and disease-specific treatment.
- Rheumatology: inflammatory or autoimmune joint disease.
- Physiotherapy, occupational or speech therapy: delivery of the rehabilitation programme according to the patient’s needs.
A PM&R physician can coordinate this pathway and refer when another specialty is the more appropriate lead.

Is a PM&R physician a real medical doctor?

Is PM&R the same as orthopaedics?
