Physiotherapist or Rehabilitation Medicine Doctor in Islamabad?

Physiotherapy and rehabilitation medicine work closely together, but they are different professions with different responsibilities.

A PM&R physician is a medical specialist; a physiotherapist is a rehabilitation professional

A PM&R physician completes medical training and specialist qualification in Physical Medicine and Rehabilitation. The physician assesses diagnosis, medical risk, medicines, investigations, EMG/NCS, selected procedures and the overall rehabilitation plan.

A physiotherapist assesses movement and delivers rehabilitation through exercise, education, manual approaches, activity progression and other modalities within professional scope. High-quality care often uses both roles rather than pretending they are identical.

Choose according to uncertainty, complexity and goals

Physiotherapy may be a direct first step

A straightforward, already-assessed movement problem, graded exercise need, post-operative protocol or sports reconditioning programme.

Book a PM&R physician first

Uncertain diagnosis, significant weakness or numbness, several medical conditions, persistent pain despite treatment, need for EMG/NCS or consideration of a procedure.

Use a coordinated programme

Stroke, spinal-cord injury, severe disability, complex sports injury, pelvic-floor problems, chronic pain and recovery after major illness or surgery.

Escalate urgently

Sudden weakness, major trauma, fever with severe pain, acute bladder or bowel loss, chest pain or other emergency warning signs.

The word “doctor” does not tell patients what clinical role they are booking

Patients frequently search “physiotherapy doctor,” “pain physiotherapist,” “rehab doctor” or “physical therapy clinic” without knowing the difference. The safest comparison is based on recognised qualification and scope, not advertising language alone.

Dr Muhammad Shoaib Mukhtar is a consultant physician with FCPS in PM&R. His role is physician-led diagnosis, pain and rehabilitation planning, EMG/NCS and selected procedures, with physiotherapy and other rehabilitation treatments coordinated according to need.

Diagnosis and rehabilitation should communicate

The physician and therapist should agree on the working diagnosis, precautions, measurable goals and progression. If a patient is not improving, the plan should be reviewed rather than repeating the same sessions indefinitely.

Frequently asked questions

Is PM&R the same as physiotherapy?

No. PM&R is a physician medical specialty. Physiotherapy is a separate rehabilitation profession. They frequently work together.

Can I see a physiotherapist without seeing a physician?

For many straightforward problems this may be reasonable. Medical assessment is more important when diagnosis is uncertain, symptoms are severe or neurological, medical conditions complicate care, or treatment is not progressing.

Does Dr Shoaib personally provide medical assessment?

Yes. The consultation, medical diagnosis, electrodiagnostic assessment and procedure decisions are physician-led. Rehabilitation sessions may involve an appropriate multidisciplinary team.
