PRP is a preparation, not a universal cure
Platelet-rich plasma is prepared from a patient’s own blood and injected after concentration of platelets. Preparation systems differ, so “PRP” is not one perfectly standard product. It may be discussed for selected people with symptomatic knee osteoarthritis, but it should not be advertised as guaranteed cartilage regrowth or a permanent cure.
The first question is whether osteoarthritis is actually the main source of symptoms. Hip pain, lumbar nerve pain, meniscal injury, inflammatory arthritis and poor strength can overlap with knee pain. Examination and appropriate imaging help establish the working diagnosis.
Who may or may not be considered?
Suitability depends on symptom pattern, arthritis severity, age, medical conditions, medicines, infection risk, blood disorders, previous treatment and goals. Patients taking anticoagulants or antiplatelet medicines should never stop them without the prescribing clinician. Active infection, an unexplained hot swollen joint and some blood conditions require particular caution.
Compare reasonable options
Education, weight management where relevant, progressive strength and aerobic activity remain central. Medication, walking aids, bracing, corticosteroid, hyaluronic acid, PRP and surgical opinion each have different indications, evidence, costs and risks. The right comparison is not “PRP or nothing”; it is which combination best fits the patient now.
What happens around the procedure?
A clinician should explain preparation, guidance method, aftercare, expected temporary flare, warning signs and when rehabilitation resumes. Ultrasound guidance can improve visualisation and placement but cannot guarantee clinical benefit. Recovery is monitored through walking, stairs, sleep and chosen activities rather than a promise of immediate pain removal.

Consent should include temporary pain flare, infection, bleeding and the possibility of no meaningful benefit. Ask what rehabilitation is expected before and after injection and when the treatment will be judged unsuccessful. An ethical plan defines the decision points before money is spent.

Bring weight-bearing knee X-rays if available, MRI only if previously performed, a medicine list, details of earlier injections and the exercise programme already attempted.
